Alternatieven voor reguliere NSAID’s bij gewrichtsslijtage en artritis

Gewrichtsslijtage en (reumatische) artritis zijn aandoeningen die de laatste jaren een epidemische omvang hebben aangenomen. De reguliere geneeskunde doet vaak alleen aan symptoombestrijding en zet hierbij met name niet-steroïdale anti-inflammatoire drugs, ofwel NSAID’s, in. Bekende voorbeelden hiervan zijn onder andere diclofenac, ibuprofen, naproxen en de nieuwere categorie COX-2-remmers zoals celecoxib of etoricoxib.

Deze reguliere geneesmiddelen, waarvan enkele zelfs zonder recept verkrijgbaar zijn, geven helaas veel problemen, met name bij langdurig gebruik of comorbiditeit. NSAID’s zijn schadelijke geneesmiddelen. NSAID’s geven namelijk veel interacties met reguliere geneesmiddelen, hebben hinderlijke bijwerkingen en zijn ook erg schadelijk voor organen zoals hart, maag en nieren. Voor hart-vaatpatiënten kunnen bestaande hartgerelateerde klachten verergeren en neemt de kans op beroertes en een myocardinfarct toe. Het verhoogde risico is vooral van toepassing op mensen die al risico lopen. Een patiënt met eerdere hartproblemen, hoge bloeddruk en diabetes heeft statistisch een kans van 5% op een hartaanval. Als deze persoon het NSAID diclofenac gebruikt, zal dit risico met 40% toenemen tot 7%. Met andere woorden één op de 50 van deze patiënten krijgt bij gebruik van diclofenac een vermijdbare hartaanval.1 Bij patiënten met bestaande ischemische klachten, perifeer vaatlijden, cerebrovasculaire bloedingen of andere cerebrovasculaire aandoeningen of hartfalen zijn de NSAID’s daarom gecontra-indiceerd. Ook is er een sterk verhoogde kans op het ontwikkelen van een ulcus en zijn NSAID’s gecontra-indiceerd bij patiënten met een bestaande ulcus, met een ulcus in de voorgeschiedenis en bij gastritis.

NSAID’s gaan daarnaast erg veel onwenselijke interacties aan bij comedicatie. De lijst is te omvangrijk om hier compleet te vermelden, maar de NSAID’s geven onder andere een verhoogde kans op toxiciteit bij combinatie met lithium, digoxine en methotrexaat, een verhoogde kans op bloedingen als ze gecombineerd worden met bloedverdunners, een verhoogde kans op ulcusvorming in combinatie met SSRI’s, en ze kunnen het effect van antihypertensiva verminderen.2 Het is daarom onveilig en zeer onwenselijk dat een belangrijk deel van de NSAID’s zoals diclofenac, ibuprofen en naproxen zonder recept verkrijgbaar is bij drogisterij, supermarkt, via internet en zelfs bij de benzinepomp!
