Rode gist: mogelijk alternatief voor reguliere cholesterolverlagers?

Zowel bij de Vitamineapotheek als in onze reguliere apotheek krijgen we frequent de vraag om een natuurlijk alternatief voor de reguliere cholesterolverlagers. In de praktijk worden statines soms slecht verdragen; vooral de hinderlijke spierpijn in de benen is een veelgehoorde klacht. Om die reden is er bij een aanzienlijke groep statinegebruikers vaak behoefte aan een alternatief dat cholesterol effectief verlaagt, zonder dat deze nare bijwerking optreedt.

De statines, ontdekt en ontwikkeld in de jaren 70, zijn vandaag de dag niet meer weg te denken uit het medische behandelprotocol. Grotendeels vanwege de grote invloed van de farmaceutische industrie die inmiddels miljarden heeft kunnen verdienen aan de cholesterolverlagers. De statines grijpen in op de cholesterolsynthese door remming van de snelheidsbepalende stap (HMG-CoA reductase.

Een statine wordt voorgeschreven bij hypercholesterolemie, ter preventie van hart- en vaatziekten en diabetes mellitus. De in de praktijk veelvoorkomende spierklachten als gevolg van statinegebruik lijken genetisch bepaald te zijn1,2, mogelijk via een mutatie in een van de genen die codeert voor de biosynthese van co-enzym Q10, dat belangrijk is voor de energievoorziening van de cel.1 Deze klachten variëren van een goedaardige spier- of gewrichtspijn tot myositis met een meer dan tienvoudige verhoging van het creatinekinase (CK), wat een indicatie is voor verhoogde spierafbraak. Ook de hartspier werkt minder goed bij statinegebruikers.3 De incidentie van spierpijn na gebruik van statines varieert van 1 tot 7 procent. Ernstige myopathie treedt op bij 0,5 procent van de patiënten. Levensbedreigende rabdomyolyse komt gelukkig zeer zelden voor (ongeveer 1 op 100.000 gebruikers). Rabdomyolyse kan optreden bij daarvoor gevoelige personen en bij gelijktijdig gebruik van andere middelen die de afbraak van een statine verhinderen, bijvoorbeeld via de metabolisatie door het CYP3A4-enzymsysteem.
Statines blijken het risico op diabetes in lichte mate te verhogen (9 procent)5 en naarmate de dosering hoger is, neemt dit risico verder toe.<sup<6 Het tegenstrijdige is dat diabetes een belangrijke risicofactor is voor hart- en vaatziekten, die men juist beoogt te bestrijden met behulp van statines. Toch wegen de cardiovasculaire voordelen in de meeste gevallen op tegen het licht verhoogde risico op diabetes.7 Daarbij is het belangrijk te vermelden dat het extra risico op diabetes door statinegebruik bij postmenopauzale vrouwen aanmerkelijk hoger is, namelijk 71 procent.8
