Veilige optie als pijnstiller en ontstekingsremmer

Het toedienen van specifieke enzymcombinaties heeft systemische effecten en wordt daarom ‘systemische enzymtherapie’ genoemd. Er worden tal van gunstige effecten geclaimd op heel uiteenlopende lichaamsprocessen. Onder meer dat ze ontstekingsprocessen natuurlijker doen verlopen, zonder het genezingsproces te hinderen.1,2

De arts John Bard beschreef systemische enzymtherapie voor het eerst aan het begin van de twintigste eeuw. Max Wolf en Helen Benitez hebben deze behandelvorm vervolgens in de jaren vijftig verder ontwikkeld.1,2 Uit onderzoek blijkt dat – na bescherming tegen maagzuur – de plasmaspiegels van de enzymen inderdaad toenemen en dat hun proteolytische activiteit meetbaar is.3 Alpha-2-macroglobuline (A2M) is de meest aanwezige anti-protease. Dit molecuul controleert ontstekingsprocessen door pro-inflammatoire cytokinen te verwijderen. De spiegel van A2M wordt verhoogd door het oraal toedienen van enzymen, zoals bromelaïne en trypsine.4
Om tot een daadwerkelijk systemische werking in het lichaam te komen is het belangrijk dat de enzymen intact worden opgenomen en dat de actieve structuur ervan tegen maagzuur wordt beschermd. Na absorptie in de dunne darm behouden zij hun werking doordat de geabsorbeerde enzymen zich binden aan anti-proteases in het bloed.

Aanvullende immuun-modulerende effecten van deze enzymen zijn splitsing van bepaalde moleculen op de celwand van de T-lymfocyt waardoor modulatie plaatsvindt en een afname van de migratie van neutrofielen naar plekken van acute inflammatie. Bovendien activeren de enzymen de protease-activated receptor 2 (PAR). PAR komt tot expressie op T-cellen, eosinofielen, neutrofielen en mestcellen. Hierdoor dragen de enzymen bij aan ontstekingsremming en modulatie van het immuunsysteem.5,6

Bij een bloeduitstorting, een verzwikte enkel of ander acuut trauma is het doel om de weefselschade veilig en snel te behandelen. Bescherming, stoppen van bloeden, rust, icing, compressie en hooghouden van een ledemaat kunnen onderdeel zijn van de behandeling. Pijn wordt meestal behandeld met paracetamol of NSAID’s, mits de pijn niet te heftig is. Het nadeel daarvan is echter dat ze een negatief effect kunnen hebben op het herstel van spieren.7,8 Hier kan enzymtherapie op een veilige manier uitkomst bieden.
